We are pleased to inform you any first responder that received Moderna 
is eligible for a Booster. The Rutland Vaccine Team will be hosting 
clinics at the Rutland Public Library located at 280 Main Street in the 
lower level. Please use the following links to schedule your 
appointment. This is a new system to register, please follow these 
instructions on booking your appointment.

Q1 - Select Clinic - COVID-19
Q2 - Doses - Two Doses
Q3 - Manufacturer - Moderna
Q4 - Date of Vaccination - Should be on or around 2/12/2021 check you card.
Q5 - Immunocompromised - Yes or No - Most will pick NO
Q6 - DOB
Q7 - YOU MUST PICK YES
